As part of your wellness programming, you want to assess the health of your employee population. Our onsite health assessment services are here to help.

 

But we don't stop there. Our services include follow-up with any high risk individuals and encourage employees to seek personalized care from their physicians.

Biometric Screenings

Biometric Screenings are a valuable resource for providing measurable data and identifying any areas of concern or gaps in care. HPA specializes in a superior biometric health screening experience. All of our biometric screenings include HIPAA compliant aggregate reporting.

Health Plan Advocate Biometric Screenings

Biometric Screenings offer assessment of:

Total Cholesterol

Triglycerides

HDL/LDL Cholesterol

Glucose

Waist Circumference

Blood Pressure

Body Mass Index

Body Fat Percentage

Cotinine (Nicotine) Swabs*

*Cotinine testing is an add-on to traditional screenings and provided on an a la carte basis.

Onsite Biometric Screenings

Here's why HPA is the right choice for your onsite health screenings:

  • Screenings are available for any of your chosen locations, are HIPAA compliant, 15 minutes in length, and screening participants will receive all results before end of screening.

  • Our screeners review results with each participant, often with year-over-year results for returning employees. This provides an in-person coaching opportunity to help members work towards improving their numbers.

  • All marketing materials needed for communication with your employees are provided, helping to make your screening a success.

  • After screenings, HPA performs follow-up calls to any employees that fall into a high risk category, providing additional resources and encouraging follow-up with a physician.

  • Onsite Concierge: HPA offers this as an additional onsite screening component. The concierge will take the work off your HR team, helping to sign-in employees, assist with paperwork, facilitate timely screenings, answer questions, help employees find an in-network physician, and more.

Physician Form Management

If you have locations that aren't big enough to accommodate onsite screenings or the onsite approach isn't right for you, HPA offers HIPAA compliant physician form management.

HPA facilitates physician forms by:

  • Creating and distributing customized physician forms that members will take to an annual physical with their doctor to have completed.

  • Collecting completed forms and compiling results

  • Taking the work off of your HR team

  • Providing extensive reporting

In-Office Screenings and Lab Partners

Health Plan Advocate Health Assessment Services

HPA also offers in-office screenings at our Grand Rapids location. Members can make an appointment and have their screening done at our office. This is a great option for members who are unable to attend onsite screenings or for new hires.

For your employees that aren't located in Grand Rapids, HPA partners with a variety of labs where members can complete their screening.

Aggregate Reporting and Compliancy

HPA provides in-depth aggregate reporting compiled from onsite screenings, in-office screenings, and physician forms. This report compares your plans year-over-year data to national benchmarks and HPA averages, allowing HPA to make program improvement recommendations to assess high risk areas and further develop your wellness strategy. You will also be kept up-to-date with a compliancy report to provide insight on which members are meeting criteria or completing reasonable alternatives when applicable.

Health Risk Assessments (HRAs)

Our health risk assessment is an in-depth questionnaire which provides the opportunity for a member to self-evaluate on topics such as nutrition, activity, mental health, and more. Upon completion, members are provided with a report which includes health recommendations and year-to-year comparative results. We offer the HRA in paper format or online through our wellness portal services. HPA also uses these assessments to identify and provide resources to any employees with mental health risks.
